By submitting this request, you:

• acknowledge that you have read and understood the above information
• accept that work may only take place in line with the conditions stated on an issued Close Approach Consent
• acknowledge that any issued Close Approach Consent is only valid for the date and times stated
• acknowledge that, unless lines have been isolated, workers must legally maintain a minimum approach distance (MAD) of at least 0.5m from overhead lines, even with a Close Approach Consent and other hazard control methods in place
• agree to abide by the directions of Wellington Electricity (we*) representatives at all times
• accept that the work party shall be liable for the cost of any damage caused to we*s lines or equipment during the work
• understand that:
  • issuance of a Close Approach Consent may be subject to charges – for example, if we need to implement hazard control measures such as sleeving or isolation (in which case, we'll advise and provide you with an estimate for your acceptance)
  • an initial on-site assessment will need to take place prior to a Close Approach Consent being granted; the issuance of which could take up to 20 working days (or longer in some circumstances) following this assessment
  • standovers for access to a controlled environment don't usually need an initial assessment but may be subject to charges, depending on the nature and duration of the access required. Any persons accessing a controlled environment will be need to supply their own personal protective equipment (PPE), which we can advise on.

By submitting this request, you:

• acknowledge that any issued High Load Permit is only valid for the date and time stated
• agree to abide by the directions of Wellington Electricity (we*) representatives during transportation
• acknowledge that you are responsible for liaising with any other overhead obstruction owners along the proposed route and for ensuring that the load is legally compliant (see below)
• accept that the haulage operator shall be liable for the cost of any damage caused to we*s lines or equipment during transportation, where they have failed to follow the conditions of the High Load Permit
• understand that issuance of a High Load Permit is subject to the indicative costs and timeframes listed below:
